Important Plan Information
Prescription Benefits. The prescription calculator illustrates potential cash benefits available under certain ManhattanLife Home Health Care Enhanced plans. For eligible 30-day prescriptions, the illustrated benefit is $10 per generic prescription and $25 per brand-name prescription. Annual prescription benefit maximums are $300 for Classic, $600 for Premier, and $900 for Deluxe. The calculator uses the Deluxe benefit level for illustration. Actual benefits depend on eligible prescriptions, claim documentation, policy provisions, annual limits, and state availability. These amounts represent potential cash benefits, not insurance premiums. Prescription benefits do not replace Medicare Part D or other prescription drug coverage.
Approval and Coverage. Some plans offer simplified underwriting or faster application decisions. Other applications may require additional review or health information. Approval is not guaranteed. Coverage becomes effective only when the issuing insurance company's applicable policy and premium requirements are satisfied.
Plan Prices. Insurance premiums depend on the insurance company, plan selected, age, state, available benefits, and applicable underwriting requirements. The prescription calculator does not calculate insurance premiums. Contact a licensed Shore Legacy agent for current quotes and available options.
Help at Home. Medicare may cover certain skilled home health services when eligibility requirements are met, but generally does not cover custodial care alone, such as assistance with bathing, dressing, or eating. Home health care insurance may provide cash benefits for qualifying covered care at home. Actual care costs vary by location and services needed.
Family Caregivers. Certain home health care plans may provide benefits when qualifying care is furnished by an eligible family member, friend, or neighbor. These benefits are subject to the policy's requirements, limitations, and state availability. Where applicable, cash benefits are paid according to the policy terms.
Plan Details. Home health care, hospital indemnity, and cancer, heart attack and stroke products have different benefits, eligibility requirements, issue ages, limitations, and exclusions. Hospital benefits, emergency room benefits, ambulance benefits, rehabilitation benefits, and optional riders are available only when included in the applicable policy. Not all products or benefits are available in every state.
Hospital and Rehabilitation Stays. Medicare may cover certain qualifying skilled nursing or rehabilitation services following a hospital stay, subject to Medicare requirements and applicable cost sharing. Supplemental hospital indemnity benefits may provide additional cash payments for covered hospital or rehabilitation stays, depending on the policy.
Images. Photographs and illustrations are for illustrative purposes only and do not represent actual insured customers or guaranteed care outcomes.
Important Coverage Limitations. Home health care, hospital indemnity, and specified-disease insurance products provide limited or supplemental benefits. They are not major medical insurance, Medicare, Medicare Supplement insurance, or Medicaid, and do not replace comprehensive health coverage. Benefits may be less than actual expenses. Policies may include waiting periods, exclusions, benefit maximums, and other limitations. Renewal provisions and premium changes depend on the applicable policy and state requirements.
Agency Disclosure. Shore Legacy Insurance, LLC is an independent insurance agency, not an insurance company. Insurance products are issued by the insurance companies we represent.
